Elevated proton relaxation times (T 1 and T 2 ), which can be measured from MR images, have been reported in various bone marrow disorders [6-8], and potentially provide an objective means of detecting focal bone marrow abnormalities and assessing response to treatment. The clinical role of quantitative MR imaging in lymphoma is, however, unclear. A means of improving the detection of bone marrow involvement by lymphoma would have important therapeutic implications. MR imaging of bone marrow may improve the accuracy of staging of some patients.
Normalisation of T 2 variation was seen in three of the four patients studied serially. Discussion Conventional magnetic resonance imaging is of value in detecting malignant marrow infiltrates [3, 5, 12] and has been shown to be superior to bone marrow scintigraphy in the evaluation of bone marrow status . A quantitative approach, however, allows an objective assessment of the presence of marrow abnormalities to be made and, importantly, provides a means of assessing treatment response. 42 Quantitative MR imaging studies have shown that the T 1 of LV bone marrow reflects alterations in bone marrow cellularity (the more cellular the marrow the higher the T 1).
4 ), site, and bulk of disease. As patients are surviving for many years after treatment for Hodgkin's disease, the long-term complications and morbidity of therapy have become more clear and of major concern. These complications are listed in Table 1. The challenge to physicians and clinical investigators of the next decade is to recognize and acknowledge the importance of these complications and refine and modify management programs to reduce them as much as possible. For patients with favorable settings of the disease and good prognoses, it is essential to reduce the acute toxicity and long-term effects of chemotherapy and irradiation programs.
